The APh Clinical Pharmacist is responsible for providing prescribed medications, drugs and other
pharmaceuticals as needed for adequate patient care, according to professional standards and
practices. In addition, the APH Clinical Pharmacist is responsible for providing patients and providers with
Therapeutic assessments and selected disease management through Medication Therapy Management program, working toward improving patient and
practice outcomes. These Pharmacists function as provider delegates with outside agencies such as pharmacies and insurances.
Responsibilities:
- Performs pharmacy clinical functions conforming to the California Board of Pharmacy (BOP) rules, regulations, and guidance.
- Provides clinical pharmacy services, including but not limited to adhering to the best clinical practices, performing comprehensive medication management, under the TrueCare Collaborative protocol.
- Provides all clinical pharmacy services to TrueCare providers and patients in a legal and ethical manner following the best and most current clinical practices.
- Routinely communicates with physicians, nurses, and patients regarding drug therapy.
- Reviews and evaluates individual drug orders for appropriateness, safety, and cost.
- Consults with patients on their prescriptions.
- Orders or performs routine drug therapy-related patient assessment procedures including laboratory tests, blood pressure, blood glucose, temperature, pulse, and respiration.
- Documents Medication Therapy Management encounters in the Electronic Health Record (E.H.R.).
- Advises the Pharmaceutical and Therapeutics Committee, as requested.
- Stays abreast with the TrueCare dispensing drug formulary as well as insurance formularies and medical resources in order to provide justifications for prior authorizations and make therapeutic substitutions.
- Stays updated on guidelines for Chronic conditions.
- Develops and updates clinical pharmacy policies and procedures annually in collaboration with the Pharmacy Management Team.
- Contributes to the success of TrueCare by participating in quality improvement activities and meets productivity goal.
Required Qualifications:
- Bachelor’s degree in pharmacy or Doctor of Pharmacy Degree (PharmD) from an accredited College of Pharmacy.
- Clinical pharmacy residency or equivalent experience.
- Current and unrestricted California State Board of Pharmacy License.
- Advanced Practice Pharmacist license.
- Two (2) years working as a pharmacist.
- Computer proficiency with Practice Management and Electronic Health Record systems, and the Microsoft Office suite, including Outlook, Word, and Excel.
Desired Qualifications:- Medication Therapy Management experience.
- Clinical pharmacy practice (inpatient or ambulatory) experience.
- Bilingual in English and Spanish.
- Knowledge of community health centers, public assistance programs, state and federal regulations, and managed care systems.

The pay range for this role is $62 - $93 on an hourly basis. If you are hired at TrueCare, your final base salary (within the pay range), will be determined based on factors such as education, knowledge, skills, and experience. In addition to those factors, we believe in the importance of pay equity and consider the internal equity of our current team members when determining any final offer. Please keep in mind that the range mentioned above is the full base salary range for the role. Hiring at the maximum of the range would not be typical to allow for future and continued salary growth.
