RN Case Manager, Bilingual Required, Hybrid Position, TrueCare

We are seeking an experienced Registered Nurse (RN) Case Manager to support our centralized care management team. This role focuses on population health, chronic disease management, and care coordination within a Patient-Centered Medical Home (PCMH) model.
Your Role & Impact:
- Coordinate care for high-risk and complex patients, improving outcomes and access
- Support population health initiatives, including chronic disease management and preventive care
- Use data and performance metrics to identify care gaps and improve patient outcomes
- Assist with transitions of care to ensure continuity and reduce fragmentation
- Address social determinants of health and support underserved populations
- Collaborate with providers, care teams, and external partners to ensure coordinated care
- Maintain accurate, timely documentation and ensure compliance with regulatory standards
What We’re Looking For:
- Bachelor of Science in Nursing (BSN) degree
- At least five (5) years’ clinical nursing and/or case management experience
- At least two (2) years in case management
- Knowledge of medical terminology, legal and technical aspects of a medical records system, HIPAA guidelines, and ICD-10 and CPT codes
- Computer proficiency, particularly with Electronic Health/Medical Record systems and Microsoft Office (Outlook, Excel, PowerPoint, Word)
- Bilingual in English and Spanish
Desired Qualifications:
- Case Management certificate
- Experience in an outpatient setting or Federally Qualified Health Center (FQHC)
- Experience with Epic Electronic Health Record system

The pay range for this role is $92,977 to $139,466 on an annual basis.
