The APh Clinical Pharmacist is responsible for providing prescribed medications, drugs and other
pharmaceuticals as needed for adequate patient care, according to professional standards and
practices. In addition, the APH Clinical Pharmacist is responsible for providing patients and providers with
Therapeutic assessments and selected disease management through Medication Therapy Management program, working toward improving patient and practice outcomes. These Pharmacists function as provider delegates with outside agencies such as pharmacies and insurances. An advanced practice pharmacist is a licensed pharmacist who has been recognized by the board, and is entitled to practice advanced practice pharmacy, within or outside of a licensed pharmacy. The advanced practice pharmacist license shall be coterminous (concurrent) with the licensee’s pharmacist license.
Required Qualifications:
- Bachelor’s degree in pharmacy or Doctor of Pharmacy Degree (PharmD) from an accredited College of Pharmacy.
- Clinical pharmacy residency or equivalent experience.
- Current and unrestricted California State Board of Pharmacy License.
- Advanced Practice Pharmacist license.
- Two (2) years working as a pharmacist.
- Computer proficiency with Practice Management and Electronic Health Record systems, and the Microsoft Office suite, including Outlook, Word, and Excel.
- Medication Therapy Management experience.
- Clinical pharmacy practice (inpatient or ambulatory) experience.
- Bilingual in English and Spanish.
- Knowledge of community health centers, public assistance programs, state and federal regulations, and managed care systems.

If you are hired at TrueCare, your final base salary (within the pay range), will be determined based on factors such as education, knowledge, skills, and experience. In addition to those factors, we believe in the importance of pay equity and consider the internal equity of our current team members when determining any final offer. Please keep in mind that the range mentioned above is the full base salary range for the role. Hiring at the maximum of the range would not be typical to allow for future and continued salary growth.
