Your Role & Impact
The Resource Case Manager will be responsible for conducting patient intake and all required paperwork for program enrollment. The Resource Case Manager will be responsible for completing comprehensive assessments in the areas of health, nutrition, psychosocial needs, goals, and home safety for each patient
What We’re Looking For
- Bachelor’s Degree in Health Care Administration, Nursing, Social Work, Psychology, Sociology, or related field.
- Minimum (2) two years’ working in a clinical setting.
- Experience with brief interventions and case management techniques.
- Experience working with low income, diverse populations.
- Patient advocate; facilitator; negotiator skills.
- Bilingual in English and Spanish.
- Computer proficiency with Microsoft Office suite products (Outlook, Word, Excel, and PowerPoint) and electronic health record systems.
- Knowledge of local and national resources related to health care.

The pay range for this role is $25.30 - $35.42 on an hourly basis based on experience and internal equity.Pay transparency: If you are hired at TrueCare, your salary will be determined based on factors such as education, knowledge, skills, and experience.
